my husband would like to uptain 1381 form but the prison he is in wont give him one...what are the steps he needs to take to get one?

If you are referring to a speedy trial motion, then that is done on pleading paper and there is no form for that motion. Your husband needs an attorney, because if he is in prison, he has already been convicted and should be looking at post trial remedies.
