Fortunately or unfortunately, there are no preprinted forms for this purpose, unless it you are speaking about a simple unlawful detainer. I urge you to speak with an attorney near you, to at least consult on where to start and what your rights and remedies are. If you have not done so, I suggest that you speak to Bryan Whipple who is a member of this forum and is located in northern California.
